The aim of the Albert Centre Middlesbrough is to set out a package of support and treatment to help address whatever your individual needs are; with the overall aim of enabling you to overcome your alcohol problem.
What is a care plan?

Based on your initial assessment your named Alcohol Case-Manager will work with you to draw up a care-plan based on your individual needs. Together you will decide what support may be required to help you overcome your alcohol problem.

Initial Assessment
A member of the team will carry out a comprehensive assessment with you at The Albert Centre. This can take 1 to 1½ hours and you are welcome to bring a friend or family member along for support if you wish. We may, in certain circumstances, be able to offer a home assessment if you aren’t able to access the Albert Centre building.
After your assessment you will be allocated a named Alcohol Case-Manager who will:
•   Be your main contact in the team and offer one to one support.
•   Work with you on your care plan.
•   Liaise with other people on your behalf.
•   Be responsible for ensuring your care plan is being implemented.

The Groups

The team run a number of different groups appropriate to a range of needs. Although the groups are small, we understand that people can be worried about attending, especially for the first time. However, groups are proven to be an effective way of helping people affect change and provide support. Ask your named worker for more information.
